PRINTABLE PACKETS WILL BE AVAILABLE FOR DOWNLOAD STARTING MONDAY, January 31st.

All printable packets can be turned into the office starting February 7th.

Be sure to include all necessary paperwork such as:

IMMUNIZATIONS

PROOF OF RESIDENCY

AUTHORIZATION TO RELEASE INFORMATION 

STUDENT HEALTH INFORMATION FORM  

SCHOOL ENTRY HEALTH CHECKUP (REQUIRED PRIOR TO 1ST GRADE)  

ORAL HEALTH ASSESSMENT (REQUIRED BY MAY 31ST OF STUDENT’S FIRST YEAR OF PUBLIC SCHOOL)
